Outreach Services/Meal Delivery Coordinator

1. Assist Director in developing and implementation of Outreach and Supportive Services to elders
2. Deliver meals to home-bound elderly
3. Perform client assessments and surveys
4. Maintain case management procedures per client
5. Maintain relevant data for reporting
6. Assess and determine individual client’s eligibility needs
7. Assist in identifying and developing relevant programs and other services to meet client needs
8. Assist cook and kitchen staff in packaging home-delivered meals and related kitchen duties such as making coffee, wrapping silverware, etc.
9. Assist in ordering, securing, maintaining and inventory necessary food and supplies
10. Assist in preparation schedule and preparation of food
11. Attend pertinent meetings and training
12. Sweep and mop dinner area
13. Sanitize, wipe down the dinner table, take out garbage and wash it daily
14. Clean and sanitize bathrooms daily, mop floors and stock with paper towels bathroom tissues
15. Wash vehicle and vacuum every Friday
16. Complete meal deliveries in a timely manner

QUALIFICATIONS:

1. Knowledge and ability to work with and relate to specific needs of the elderly
2. Excellent communication and organizational skills
3. Valid CA Driver’s License
4. Must be able to pass drug test, Physical Exam and Background Check
5. Must have great teamwork skills
